primary accountability for the development, coordination and
management of the patient's treatment plans in order to achieve

Functioning as a leader for coordinating care efforts, the Case
Manager works with payor sources, patients and team members to
develop, deliver and monitor the quality of services provided and
to improve outcomes through evaluation and revision of those
services.2. With the Nurse Manager, efficiently manage the care of
the patient to promote optimal length of stay outcomes.3. Case
Manager shall manage groups of patients based upon service line
designation throughout the continuum of care. This includes
pre-hospitalization through post hospitalization4. Case Manager
shall evaluate patients referred for home care. Plans and
coordinates care for Home Care patients utilizing knowledge of
State, Federal and JCAHO regulations. Identifies insurance coverage
and communicates coverage to patient/ family. Collects, maintains
and reports statistical data on all referrals. Provides ongoing
information on home care programs and services.5. Case Manager
follows up on complex cases and communicates with the patient/
family so to insure excellent quality of care continues once
discharge has occurred.6. The Case Manager uses his/ her skills to
incorporate: assessment, planning, implementation, coordination and
continuous evaluation of a patient on a daily basis.7. The Case
Manager insures that patients have their physical, psychosocial and
vocational health needs met along the continuum of care.8. Other
duties and responsibilities as requested by Manager, Case
Management Review. QualificationsEDUCATIONGraduate of an accredited
school of nursing and currently licensed by the State of
Connecticut. BSN and Masters degree in Nuraing or Health Care
Administration preferred.SPECIAL SKILLSRecent appropriate nursing
experience, theoretical knowledge of the nursing process, case
management and continuity of care. Advanced communication and
interpersonal skills with all levels of internal and external
customers. Ability to obtain and interpret information appropriate
to patient needs and age. Advanced assessment and teaching skills.
Leadership skills in planning and managing patient care as acquired
through greater than 3 years of clinical nursing experience in an
acute care hospital. Utilization management and case management
experience preferred. Knowledge of computer software and hardware
applications and a basic knowledge of statistics.PHYSICAL
DEMANDOffice, hospital units, on call for weekends required and
will be rotated among the nurse case management staff. This may
evolve to a part time weekend schedule if patient work load
requirements mandate on-site coverage. 50% sedentary; sitting,
standing, walking from unit to unit, carrying records, speaking
before groups. Must be able to speak and hear in a manner
understood by most people. Must be able to communicate effectively
