Responsibilities
1. Clinical Practice1 .1 Manages incoming referrals, coordinates
initial patient contact, and complies list of presented patientsin
collaboration with attending physicians, fellows, and Advanced
Practice Provider, (APP's).1 .2 - -Facilitates collaboration and a
team approach in the development and implementation of each
patients - - - - -plan of care.1 .3 Performs ongoing assessment and
management of care.1 .4 Maintains ongoing communication with team
members; in collaboration with the attending physicians - - - -and
fellows conveys pertinent medical information to primary and
referring physicians.1 .5 Supports patients in the ambulatory
setting.1 .6 Assists with management and education related to
devices, pleural drains, and dressings.1 .7 - Conveys information
pertaining to hospital policy and procedure to resident staff
faculty and other - - - -members of the service team.1 .8 Coaches
patient/family through diagnostic and therapeutic experiences to
minimize anxiety and ensure - - - -adherence to medical plan;
provides warm handoffs as appropriate and maintains contact with -
- - -patient/family and point of care managers as patients move
along the continuum.1 .9 Responds to patient/family telephone
contacts according to assessment findings, physician direction - -
- -and/or established standards of care; prescription renewal or
initiation requires a physician signature.1 .10 Functions as a
clinical resource for staff and patients.1 .11 Assumes all other
duties as requested.1 .12 Data collection and analysis for IV
related data, blood transfusions and central line care1 .13 CLABSI
education and prevention. Involvement in nurse shared leadership
council.2. Clinical Program2 .1 Assists in the coordination of the
IV and PICC program.2 .2 Interacts with internal and external
contacts regarding the clinical program.2 .3 As requested,
communicates results with patients and referring physicians.2 .4
Responds to patient questions and telephone inquiries.2 .5 Provides
education and instructions to patients and families as
appropriate.2 .6 Follows program guidelines related to triaging,
scheduling, patient calls, and follow up.2 .7 - As requested,
assists with scheduling and coordination of diagnostic testing,
consultations, andprocedures.2 .8 Facilitates the integration of
documentation and information systems.2 .9 Serves as the primary
contact for patients and families.2 .10 Coordinates care of
multiple providers across the continuum of care.2 .11 Improves
patient preparedness for care/treatment through education and
psychosocial support.2 .12 Facilitates interactions between
patients, their physicians, and referrals as appropriate.2 .13
Assists in the development of appropriate educational materials and
program monitoring to promote - - - - coordination of care and
patient satisfaction.2 .14 Assists the clinical and leadership team
with program development, expansion, and successful - - - -
-achievement of programmatic goals.3. Patient Safety, Quality, and
Research3 .1 Creates and supports a culture and environment of

identifies and takes action to resolve demonstrated - - -
-discrepancies; manages patient complaints in a timely manner and
in collaboration with Patient - - - -Relations, Program Manager,
and Medical Director. Accurately identifies, and as appropriate

Director.3 .3 Participates in and/or facilitates safety and quality
improvement efforts3 .4 Fosters academic and clinical investigation
through participation in research projects in the specialty - - -
practice area and related fields of study. Participates in
literature review, data collection and data entry.4. Professional
Development and Education4.1 Participates in enhancing the
professional development of the nursing staff and other health care
- - -providers involved in the care of IV patients.4 .2 Assesses
educational needs of staff and provides in-service education as
required.4.3 Collaborates with the Clinical Nurse Specialists and
nursing staff on the development of Patient Care - - Standards and
Clinical Pathways related to the care of IV patients.4 .4
Collaborates with the nurse in assessing the learning needs of
patients and their families; provides - - - -education and support
as appropriate.4 .5 Provides information to patients and families
pertaining to disease processes and/or procedures; - - - -develops
teaching tools as required.4 .6 Provides patient/family instruction
related to disease, treatment, potential adverse effects, symptom -
- -identification and management.4 .7 Functions as a resource to
staff nurses and other health care members providing education and
clinical - - support.4.8 Collaborates with physicians and nursing
on the development of disease specific patient care standards - -
-and protocols. -
Qualifications
EDUCATION (number of years and type required to perform the
position duties):Bachelor's Degree in Nursing preferred.
Connecticut RN licenseEXPERIENCE(number of years and type required
to meet an acceptable level of performance):A minimum of three (3)
years' experience in IV therapy; PICC preferred. Certification also
preferred.SPECIAL SKILLS:Knowledge of IV access physiology and
disease processes. Excellent collaborative, communication,
andinterpersonal skills required. Ability to triage non-urgent and
emergent situations.ACCOUNTABILITY (how this position is held
accountable for such as goals achievement, budget adherence,
orother areas of accountability):Understands the importance of
careful and thoughtful follow through on issues related to IV
therapy and PICC care. .The coordinator is accountable for
collaboration and consultation with the program 's faculty,
providers, and clinicalstaff. Is responsible for the ongoing
education and support of patients and families that receive care
through the IVteam.COMPLEXITY(describe planning, problem solving,
