quality care and nursing services under the direction of a
Registered Nurse.#INDNURSESResponsibilities:Supports organizations
mission by striving for excellence in all aspects of their job with
a focus on positive interpersonal relationship with
co-workersEvaluates the needs of patients with life limiting
illnesses and their families and provides nursing care services as
directed by the Registered Nurse and as described in the
interdisciplinary plan of careAssists client to follow preventative
and rehabilitative nursing proceduresMonitors quality and
appropriateness of services providedEstablishes and maintains
professional working relationship with health care providers and
other members of the disciplinary teams in providing patient and
family careAdheres to agency infection control and safety
policiesAdheres to the organizations policy in regards to
absenteeism and appearanceQualifications:Graduate of an NLN
accredited LPN program with current NJ LPN licensure2+ years of
med/surg and acute care nursing experience1-2 years of homecare and
hospice experience preferredExcellent verbal and written
communication skillsStrong computer skillsExcellent organizational
and interpersonal skillsCar and valid drivers licenseWillingness
and ability to travel as neededWorking Conditions/Physical Demand:
Working indoors and outdoors with clinical equipment and use of
