Park Avenue Extended Care is seeking a compassionate and skilled
Occupational Therapist to join our dedicated team of healthcare
professionals. As an Occupational Therapist at Park Avenue, you

initial and ongoing assessments of patients' physical, emotional,
and cognitive abilities to develop personalized treatment plans •
Develop and implement occupational therapy interventions to improve
patients' functional abilities, such as ADLs, mobility, and
cognitive skills • Collaborate with other healthcare professionals
to ensure comprehensive care and achieve optimal patient outcomes •
Provide education and training to patients, families, and
caregivers on compensatory strategies and adaptive techniques •
Maintain accurate and timely documentation of patient progress and
treatment plans • Participate in interdisciplinary team meetings to
discuss patient care and share knowledge and expertise
Requirements: • Currently licensed and certified as an Occupational
Therapist (OTR/L) in the state of NY • Possess excellent
communication and interpersonal skills • Ability to work
effectively in a fast-paced environment and prioritize tasks
accordingly • Strong analytical and problem-solving skills •
Familiarity with electronic medical records and therapy
documentation software • Ability to lift, push, or pull up to 50
