Job Description Job Description Job Title: Wound Care Nurse (RN) Location: Brooklyn Job Type: Full-Time Setting: Skilled Nursing / Long-Term Care Compensation: $115,000 – $120,000 annually Position Summary We are seeking a clinically strong Wound Care Nurse (RN) to lead skin integrity initiatives in a high-acuity Skilled Nursing facility. The RN will be responsible for wound assessment, treatment planning, documentation accuracy, and regulatory compliance. This role directly supports resident outcomes, survey readiness, and overall facility quality metrics within a supportive long-term care environment. Key Responsibilities Conduct comprehensive skin assessments upon admission and routinely thereafter Stage and document pressure injuries in accordance with NPIAP guidelines Develop and oversee individualized wound treatment plans Perform and supervise advanced wound care treatments and specialty dressings Lead weekly interdisciplinary wound rounds Collaborate with MDS staff to ensure accurate coding and compliance Maintain survey readiness related to skin integrity standards Track wound care metrics and participate in QAPI initiatives Educate nursing staff and CNAs on prevention protocols and documentation standards Collaborate with physicians, nursing leadership, rehabilitation, dietary, and infection control teams Qualifications Active, unencumbered RN license in New York Minimum 2 years of wound care experience in a Skilled Nursing or Sub-Acute setting Strong knowledge of pressure injury staging and prevention practices Familiarity with MDS 3.0 and Quality Measures Experience with EHR systems (PointClickCare or SigmaCare preferred) Wound Care Certification (WCC, CWCN, or similar) preferred Benefits Comprehensive medical, dental, and vision coverage 401(k) with company match Paid time off and holidays Tuition reimbursement and professional development opportunities Supportive long-term care environment with career growth potential This position is ideal for a proactive, clinically confident RN who wants to make a meaningful impact on resident care in a Skilled Nursing facility in Brooklyn, NY.
