  • Location: 501 Southern Boulevard, Bronx, NY 10455Responsibilities
    • Ensure appropriate supervision, health, and safety of individuals; implement Individual Plan of Protective Oversight.
    • Advocate for individuals in the community (medical appointments, church, recreation activities, etc.).
    • Implement and monitor individual care plans.
    • Administer medication as prescribed.
    • Assist with daily living skills such as personal hygiene, food preparation, housekeeping, laundry, and shopping.
    • Maintain facility compliance with OPWDD and COA standards.
    • Facilitate learning and skill training, including fire safety.
    • Document individual goals and progress.
    • Complete daily progress notes and logs.
    • Maintain certifications in CPR/First Aid, SCIP, and AMAP.
    • Create a positive environment to promote good behavior.
    • Observe and report behavioral changes; collaborate with clinical staff.
    • Plan and prepare balanced meals.
    • Clean and maintain the residential unit.
    • Assist individuals with shopping for groceries, clothing, hygiene supplies, and other needs.
    • Participate in workshops, trainings, and meetings.
    • Support spiritual development and uphold ethical and moral values.
    • Transport individuals for activities, appointments, and visits.
    • Assist in developing social skills.
    • Remain on shift until relieved.
    • Perform additional related tasks as needed.Qualifications
      • AMAP, SCIP, CPR, and First Aid certifications preferred.
      • Valid NY/NJ/CT/PA driver's license with 6 points or less required.
      • Experience working with adults with developmental disabilities.
      • Strong commitment to safety.Education RequiredHigh School diploma or G.E.D.
