Job Description:
A healthcare organization is seeking a Service Plan Writer to
create and manage personalized care plans that support seniors
living safely at home. Responsibilities Include: Develop and update
individualized service plans Review client information and medical
details for accuracy Ensure plans meet regulatory requirements
before submission Maintain organized documentation across multiple
cases Collaborate with Service Coordinators for accuracy and
efficiency Ideal Qualifications: Strong writing and analytical
skills High attention to detail and organization Ability to manage
multiple deadlines Strong communication and time management skills
Experience in healthcare or case management preferred Salary: $50k
